The NHS in Lewes, in a partnership with the three Lewes GP practices, is providing FREE one-to-one support for people registered with a GP in Lewes. If you have a long-term condition such as diabetes, chronic pain, heart disease, arthritis, etc. you will be able to get free health coaching support.

At the Council’s suggestion I recently had a meeting with John Worth of Know Your Own Health which is behind the Live well, feel better campaign in Lewes.
Know Your Own Health receives funding from the Clinical Commissioning Group (CCG ) and offers free health coaching sessions for people registered with a GP in Lewes and who have a long term physical health condition. He says the service is aimed as those who feel “stuck” with their condition; lack confidence, and lean on their GP more than necessary. They offer up to 5 hours of one-to- one counselling and then stay in touch by e-mail.
Their aim is to reduce over-reliance on GPs and to reduce unplanned hospital admissions by helping the person to articulate their needs more clearly to their GP. They also want to help those who are newly diagnosed with a long term condition who may be in a state of shock or bewilderment.
Unfortunately there is not a similar service for those with long term mental health conditions.
Users of the service are usually referred by their GP, but you can self-refer if you wish. So, if you think you might benefit from this service, or know anyone else who lives in Rodmell who might, please let them know.
